C & S Care Management is owned and operated by Cindy Hartman, RN. With over 25 years of care management experience, Cindy acts as an advocate for her clients. She offers helpful options and solutions to clients and their families when difficult decisions regarding care and services present themselves.

C & S Care Management is needed when:

Clients are no longer able to live independently.
A loved one is having problems, wants to remain in the home but needs help with the activities of daily living (ADL's).
You live out of town and need help in arranging care for a loved one that lives away from you.
A family member, or someone you know, is no longer capable of managing their financial affairs, and requires assistance in this or other areas of daily living/management.
You need information about care options and placement possibilities.
You are overwhelmed with your care giving responsibilities and need assistance or are considering placement options.

Using community resources, C & S Care Management helps coordinate services and identify placement options for their clients. Cindy regularly monitors services, identifies appropriate providers/referrals, and alerts families and caregivers to potential problems, assisting in resolution of issues.

What is Care Management?

Care Management offers client advocacy, assistance in facilitating communication with providers, and identification of services and resources. A Care Manager may recommend locations of care for the client and communicate with other allied professionals regarding the client's needs.

Care Managers are professionals that act as consultants to clients and their families. These experienced professionals assist the client and family with issues surrounding the possibility of long-term care needs.

Working with the individual, family and caregiver, C & S Care Management provides appropriate service at a reasonable fee.
